Posterior Chain Exercise: most of the therapists I know are in agreement that hamstrings or the posterior chain in general is very stubborn for neurologic cases even so for the elderly population. For the elderly, it is almost always weak, for muscle tone abnormalities, strength ranges from 0/5MMT to just palpable muscle contraction. The issue causes postural instability in both static and dynamic movements. I tried and experimented with a lot of exercises and some of them works for the elderly but nothing much for neurologic cases. This is my go to exercise for the posterior chain that is hamstring dominant. I got more muscle activation from this exercise according to my patients, the result is immediate and gratifying as well. To therapists, please be conscious of the back and neck posture when performing this exercise. I set the patient standing with back against the wall, heels-shoulders-head preferably touching the wall. I usually put rolled towel on the back of the knees for a slight bend. I hold on to the thighs for stabilization. Cues are as follows: Stare at one point in front of you, keep staring all through out the exercise. Cross your arms to your chest. Keep your buttocks touching the wall. Bow slowly, go down as low as you can go. Keep your buttocks and low back tight. Go back up to starting position. Repeat.
